Berklyn Medical Solutions has scheduled courses that are reserved for CHW-East Valley Employees. On the course calendar, CHW courses are designated with having “CHW” after all course names. If you find that a particular “CHW” course is already full, you are welcome to register for any available course that fits your personal schedule more effectively.

Employees who do not receive a CHW paycheck will be required to self-pay for classes (i.e. travelers, agency personnel, and contracted employees such as physicians). Employees, who are paid directly by CHW, will not be charged for classes if the classes are required for their job. For employees that are considered to be “self-pay,” all course prices will be listed on the course sign-up page and will be paid in full at time of registration.

All CHW employees must complete the online registration process. Upon completion of the course, all billing and personal information will be deleted according to the Berklyn Medical Solutions privacy policy. If an employee does not cancel or reschedule according to the current refund policy, the employee will forfeit their deposit.

At this time, all CHW employees must provide the following deposit to the mailing address below, via personal check within 5 business days of completing the online registration to complete the reservation process. If your deposit is not received, your registration may be canceled.

• BLS-HCP (CPR): $25.00
• BLS Online Course: $25.00
• ACLS/ ACLS-R/PALS/PALS-R: $50.00
• ACLS-C: $75.00

Note: Upon completion of the course, the personal check will be given back to the employee

The MANDATORY textbooks are not included and must be brought with you to all portions of the course. CHW East Valley employees may check out a textbook for no charge from the education department at either Chandler Regional Medical Center or Mercy Gilbert Medical Center. Any textbook that is purchased from Berklyn Medical Solutions will be billed directly to the employee.

All pre-course work, if indicated, must be finished prior to the first day of the course.
